Why it stays broken
A remediation project generates 3 kinds of obligation at once: what leaves the site, who is qualified to handle it, and what must be sampled and reported. Each is managed by a different person in a different system, and the closure file is assembled at the end from whatever survived.
A single load rejected at the disposal facility sends 340 tons back to a site that is no longer set up to receive it.
